NHS band 6 pay and jobs

A band 6 job in England pays £39,959 a year from 1 April 2026. After two years you move to £42,170, and after a further three years to £48,117, so reaching the top takes five years. Band 6 suits experienced registered staff who lead a shift, hold a caseload of their own or work in a specialist area.

The three band 6 points

Step England Inner London Outer London Fringe Scotland
Entry £39,959 £47,951 £45,953 £41,957 £43,169
After 2 years £42,170 £50,604 £48,307 £44,279 £45,070
After 5 years £48,117 £56,863 £54,254 £50,387 £52,603

All figures are full-time annual pay for 2026/27. The England basic rates come from the NHS Employers pay scales and include the 3.3% award from 1 April 2026. The London and fringe columns are worked out from the High Cost Area Supplement rules. At band 6 the caps start to bite: in Outer London the supplement reaches its £6,137 maximum from the middle step, and at the top step all three zones are capped (£8,746 inner, £6,137 outer, £2,270 fringe). The supplement guide explains the percentages and limits.

Scotland’s scale is separate and higher at every point, under a two-year deal that gives 3.75% for 2026/27.

Jobs paid at band 6

Band 6 posts usually expect several years of post-registration experience, or a specialist qualification on top of registration. Common examples:

  • senior staff nurse, junior sister or charge nurse, deputy ward manager
  • clinical nurse specialist in a defined area, such as diabetes or tissue viability
  • health visitor, district nurse and school nurse after specialist community public health or community training
  • midwives working at a senior or consolidated level
  • senior physiotherapists, occupational therapists, dietitians and speech and language therapists
  • paramedics in many ambulance services
  • specialist biomedical scientists and senior pharmacy technicians
  • service and project managers in admin, finance, HR and IT

The level of responsibility makes a job band 6: supervising other registered staff, making clinical decisions within a specialty, or managing a small team or budget. Job evaluation sets the band, so titles alone can mislead.

How you get to band 6

Most people apply for band 6 after time at band 5. No rule says how long that must be, but adverts often ask for evidence of leadership on shift, teaching students, audit work or a post-registration course. Some trusts run rotational or development posts that start at band 5 and move to band 6 once set competencies are signed off.

Agenda for Change bands do not overlap, so promotion from the top of band 5 (£39,043) takes you to the band 6 entry point of £39,959. If you were still at the band 5 entry step, the jump is larger. The career progression guide covers the wider path.

What comes off band 6 pay

All three band 6 points fall in the 9.8% tier of the NHS Pension Scheme, which covers pensionable pay from £35,156 to £52,778. Basic pay at every step is below the £50,270 higher rate threshold, so income tax is at 20% on basic pay outside Scotland. London staff on the higher steps, or anyone with regular enhancements and overtime, may go over it. In Scotland the band 6 rates fall in the intermediate and higher bands of Scottish income tax, which start at £29,527 and £43,663.

The take-home pay calculator shows net pay for any band 6 salary, including student loan repayments.

Enhancements and extra pay

Band 6 clinical staff on shift rotas receive unsocial hours enhancements for nights, weekends and public holidays. The percentages for band 4 and above are the same across those bands, so the cash value rises with your basic pay. On-call payments apply where you are on a formal on-call rota. Bank shifts are paid through your trust’s bank, which NHS bank work explains.

Band 6 in the rest of the UK

Scotland’s band 6 has three points for 2026/27: £43,169, £45,070 and £52,603. Each is higher than the English equivalent, though Scottish income tax takes a bigger share at this level because the 42% higher rate starts at £43,663.

Wales announced a 3.3% award for 2026/27, matching England, according to the RCN. Northern Ireland’s award was still unconfirmed on 16 October 2026: the pay review body recommended 3.3% and the Health Minister wants to pay it, but it depends on the Executive’s budget.

If you are thinking about moving between nations, compare the full package: basic pay, tax, the pension scheme (Scotland’s has different contribution tiers) and the cost of housing where the job is.

Moving to band 7

Band 7 starts at £49,387. The usual band 7 roles are ward manager, team leader in a community service and advanced or highly specialist practitioner. Moving up normally needs a track record of leading a team or service and, for advanced clinical roles, a master’s level qualification. The band 7 guide covers what those jobs ask for. Band 6 vacancies for nurses, midwives and therapists are listed on the nursing and midwifery and allied health boards.
