NHS band 7 pay and jobs

Band 7 starts at £49,387 a year in England from 1 April 2026. It rises to £51,932 after two years and to £56,515 after three more. Ward managers, team leaders and highly specialist or advanced clinicians are typically on band 7, and most of the band sits above the higher rate tax threshold.

Band 7 pay with London supplements

Every band 7 point earns the maximum High Cost Area Supplement in all three zones, because the percentage would come to more than each cap. London band 7 pay is therefore easy to work out: add £8,746 for Inner London, £6,137 for Outer London or £2,270 for the fringe.

Step England basic Inner London Outer London Fringe Scotland
Entry £49,387 £58,133 £55,524 £51,657 £52,769
After 2 years £51,932 £60,678 £58,069 £54,202 £54,784
After 5 years £56,515 £65,261 £62,652 £58,785 £61,377

The England rates include the 3.3% consolidated award paid from 1 April 2026. The London columns are basic pay plus the capped supplement. The Scotland figures come from the NHS Scotland pay circular for 2026/27, which gives a 3.75% rise and a possible inflation top-up in March 2027. See the supplement guide for how the zones are defined.

The jobs

Band 7 roles fall into two broad groups.

Leadership posts carry responsibility for a ward, unit or community team: ward manager or sister, charge nurse running a department, community team leader, and service managers in admin, estates or finance. You manage the rota, the budget for your area, recruitment and staff performance, often while still working clinically.

Senior clinical posts need a high level of expertise without necessarily managing people. Examples are highly specialist physiotherapists and occupational therapists, sonographers, high-intensity psychological therapists, nurse specialists with an independent caseload, physician associates and trainee or developing advanced practitioners. Many advanced clinical practitioners start at band 7 while completing an MSc and move to band 8a once they qualify, although this varies by employer.

Tax and pension at band 7

Outside Scotland, the higher rate of income tax (40%) starts at £50,270. The band 7 entry point sits just under it, while the middle and top points are above it, so a slice of your pay is taxed at 40%. National Insurance drops from 8% to 2% on earnings above the same £50,270 limit. The income tax guide explains how the bands stack.

The NHS Pension Scheme is a net pay arrangement for most members, so your contribution comes off before tax is worked out. At £49,387 and £51,932 the contribution rate is 9.8%. At £56,515 it is 10.7%, the tier for pay from £52,779 to £67,668. The rate applies to the whole of your pensionable pay. The pension tax relief guide covers how that interacts with higher rate tax.

In Scotland, band 7 pay sits in the 42% higher band, which starts at £43,663. If you have a student loan, repayments continue at 9% of pay above your plan threshold. The take-home pay calculator puts these together.

What the job asks of you

Band 7 is often the first band where you are judged on what your team delivers. Ward and team managers handle sickness absence and recruitment, sign off rotas against a staffing budget, deal with complaints and incidents, and represent the area at meetings with matrons and service managers. Many still work clinical shifts as well, which can make the hours long and hard to protect.

Senior clinical posts bring a different pressure: complex cases, supervision of junior staff, teaching, audit and, in advanced practice, decisions that would once have been made by a doctor. Most require registration with the NMC or HCPC and evidence of continuing professional development.

If you are weighing up a move from band 6, ask at interview how much protected management time comes with the post, how big the team is, and what support exists for new managers. Those answers vary far more between trusts than the pay does.

Band 7 in Wales and Northern Ireland

The Welsh Government has announced 3.3% for 2026/27, as reported by the RCN. In Northern Ireland the 3.3% recommended by the pay review body had not been confirmed by 16 October 2026, because it depends on the Executive’s budget. Until a decision is made, band 7 staff in Northern Ireland stay on the previous year’s rates.

Progression within band 7 and beyond

Reaching the top of band 7 takes five years: two at entry and three at the middle point. Each step depends on your employer’s pay progression standards, usually a current appraisal, completed mandatory training and no formal capability or disciplinary process. The pay progression guide has the detail.

Band 8a begins at £57,528, just above the band 7 top point. Moves to 8a are typically into matron, service manager, consultant therapist or fully qualified advanced practitioner roles. The band 8a guide covers them. For band 7 vacancies, look at the nursing and midwifery and allied health boards.

Common questions

Is band 7 a management band?

Partly. Many band 7 posts manage a ward or team, but others are senior clinical roles with no line management, such as highly specialist therapists or sonographers. The band reflects the level of responsibility, which can be clinical or managerial.
