NHS band 8a pay and jobs

NHS band 8a pays £57,528 a year in England from 1 April 2026. After two years the rate becomes £60,417, and after three more it reaches £64,750. It is the first of the senior bands: matrons, service managers, fully qualified advanced practitioners and the first rung of consultant-level allied health roles all sit here.

Step England Inner London Outer London Fringe
Entry £57,528 £66,274 £63,665 £59,798

After 5 years £64,750 £73,496 £70,887 £67,020

England’s figures include the 3.3% consolidated rise from 1 April 2026 and are full-time annual pay. In every London zone the High Cost Area Supplement is at its cap for band 8a (£8,746 inner, £6,137 outer, £2,270 fringe), so the London columns are simply basic pay plus the cap. The supplement guide shows where the caps come from.

In Scotland, band 8a runs from £65,032 to £70,202 for 2026/27 under the NHS Scotland pay circular. Scotland’s deal gives 3.75% this year, with a possible top-up in March 2027 if average inflation for 2026 is above 2.75%.

Who works at 8a

Accountability for a service or for a specialist area of clinical practice is what the 8a roles share.

On the management side, 8a covers matrons with responsibility for a group of wards, general and operational managers for a specialty, and heads of smaller corporate functions such as a patient experience team or an information team. You may hold a budget and line manage band 7 leaders.

On the clinical side, many advanced clinical practitioners sit at 8a once they have completed their training. Qualified clinical psychologists often progress to 8a after a period at band 7, and principal pharmacists, lead sonographers and senior specialist therapists also appear here. Some consultant allied health professional posts (consultant physiotherapist, consultant radiographer) start at 8a, although many are banded higher.

Project and programme managers running larger pieces of work, and senior posts in finance and HR, are also frequently 8a.

Getting into band 8a

There is no single qualification for 8a, but adverts usually ask for a postgraduate qualification or equivalent experience, evidence of leading a team or service, and experience of budget or performance management. For clinical posts, an MSc in advanced practice or a specialist field is often essential.

Interviews for 8a posts often include a presentation on a service problem or a plan for your first months in post, as well as a panel interview. Some trusts add a stakeholder panel of staff or patients. The person specification tells you which criteria are scored at shortlisting and which at interview, so map your evidence against each one.

The bands no longer overlap, so moving from the top of band 7 (£56,515) takes you to the 8a entry point of £57,528. The rise looks small if you are already at the top of band 7, though the 8a scale has room to grow by more than £7,000 over five years.

Tax and pension at 8a

All three 8a points are above the £50,270 higher rate threshold, so part of your pay is taxed at 40% in England, Wales and Northern Ireland. In Scotland the whole 8a range falls in the 42% higher band, which runs from £43,663 to £75,000.

The NHS Pension Scheme contribution rate is 10.7% at every 8a point (the tier for pay from £52,779 to £67,668). At this level, a big pay jump or added years of pension can push the growth in your pension close to the annual allowance. The annual allowance guide explains the test, and for personal decisions the NHS Business Services Authority or a regulated financial adviser can help.

The take-home pay calculator shows net monthly pay for any 8a salary.

Flexible working and leave at senior level

Senior roles are often assumed to be full time, but flexible working is a day-one right for all employees. You can make two requests in any 12 months, and your employer must decide within two months (including any appeal), consult you before refusing, and give one of eight business reasons if it says no. Part-time and job-share arrangements at 8a are common in clinical leadership and corporate posts.

Annual leave under Agenda for Change is the same as for other bands: 27 days on appointment, 29 after five years and 33 after ten years of NHS service, plus 8 public holidays, pro rata for part-time staff. Notice periods at this level are set by your contract and are usually longer than the statutory minimum the employer must give, which is one week per complete year of service up to 12 weeks.

Moving beyond 8a

Above 8a come bands 8b, 8c and 8d. Band 8b starts at £66,582, just above the 8a top point. These bands cover heads of service, deputy directors and consultant-level clinicians with wider remits. The guide to bands 8b, 8c and 8d covers them, and senior vacancies are on the nursing and midwifery and allied health boards.

Common questions

Do band 8a staff get paid for unsocial hours?

Agenda for Change unsocial hours enhancements apply to all bands, including 8a, when you work unsocial hours as part of your contracted pattern. In practice many 8a roles are office-hours management jobs, so it depends on the post.
